Large events require a minimum of 30 days advance notice . Step 5: Provide Current Insurance Liability Form before your first use date. Note: Facility use will not be allowed without current liability insurance on file with the School District of Superior. All individual/group/organization are required to have a certificate of insurance with a minimum of $500,000 liability coverage that listed the School District of Superior as Additional Insured for all persons and activity/events. Step 7: Wait for email of approval/denial of your facilities request. To check on the status of your request, log in to your account and click on the request history tab. Step 8: When a request has been approved, an electronic contract will be sent to the requester. Sign the electronic contract with the pin you created in your account. District Facilities Use Requirements Each group requesting the use of District facilities must use the services of a District custodian/engineer and must pay for such services. Each group requesting the use of District facilities must indicate an individual, satisfactory to the administrator in charge of the building, who will serve in a supervisory capacity during the use of the District facilities. If the supervisor, as designated above, is not satisfactory to the administrator in charge, the administrator shall appoint a supervisor and the cost shall be charged to the sponsoring group. Supervisors are responsible for the enforcement of all rules and procedures regarding the use of the District facilities. The custodian on duty is directed not to open the facility until the supervisor for the sponsoring group is on duty. A certified lifeguard must be on duty at all times to supervise when the pool at Superior High School is occupied. The list is not exclusive, as other factors may require further requirements.
